Thank you for purchasing Microsoft® Money. You may be eligible to receive a rebate by mail. To receive your rebate, simply fill out this form and return it along with required proofs of purchase to Microsoft at the address on this form. Offer Expires January 1, 2009. This completed rebate form and qualifying proofs of purchase must be postmarked within 30 days of the date you download your qualifying Microsoft Money product. Valid in the 50 United States, D.C., and all US military bases with valid APO/FPO addresses. Offer may not be combined with other Microsoft offers or rebates. Limit 1 product

To receive your Microsoft Money mail-in rebate, please follow each of the steps below.

1. Download directly from Microsoft or a qualifying retailer one of the Microsoft Money Plus products listed below.
   Note: Only downloadable Money Plus products that message these rebates at the time of purchase qualify for this offer

3. To submit your claim:
   • Go to http://rebatestatus.microsoft.young-america.com and complete an online rebate form and then print and mail the rebate form with qualifying proofs of purchase to the address listed on the website, OR
   • Complete and mail this original form and send with qualifying proofs of purchase to the address listed on this form. All rebate requests must be submitted in the same envelope.
   
   
   
4. Enclose proof of purchase of the Microsoft Money Plus Product download you just purchased. Eligible proof of purchase is your purchase confirmation email for the Microsoft Money Plus Product download you just purchased.
